BMS Color Guard

Physicals

All BMS Color Guard students must have a physical before they are allowed to participate in class. If you have already done this for athletics, you do not have to complete it a second time. However, you must bring a copy of your physical form to Mr. Esparza in order for us to check. Find all 3 forms below.

Form #1: Physical
The physical can be completed at your doctor's office or at the event held on campus in the spring. To comply with UIL regulations, every band student must have an annual physical. Color guard members who are also student athletes will only complete one physical for both organizations. 

Form #2: Athletic Participation Form - Online only
All boxes at the top of this form (#1-9), even if they do not pertain to band must be checked in order for the system to process the form. Student athletes only have to complete this once, as it will work for both organizations.

Form #3: Emergency Form - Online only- Student athletes only have to do this once, as it will work for both organizations.

There will be a $70 guard fee for Beginning Color Guard and a $120 fee for Advanced Color Guard that covers the purchase of your uniform for performances and other materials needed for the class. This can be paid via Skyward Finance. Click the link to pay online, please go here.
